Creating an email form is better done using two methods available to all UW employees.
Google Forms:
This is a website I created for help in creating Google Forms. https://sites.google.com/a/wisc.edu/use-cases-for-google-forms/
Both of these will collect your data into a spreadsheet, allow you to download it, and allow you to configure the forms to notify you on submissions.
