IMPORTANT NOTICE REGARDING STUDENT ATHLETEs

A physical is required for all players in order to participate in any JH, JV or Varsity Sport. At this time we do not have a Physician to administer physicals. It is your responsibility to obtain a physical for your child. Each athlete must have a physical before they can begin practice for their particular sport. Football begins Monday, August 7. All other sports start practice on Monday, August 14. Physical forms can also be found HERE.

Your doctor will need to complete Section 6 of the PIAA Physical Evaluation Form. Parents and athletes must complete and sign section 1 thru 5 of the Physical form. If you have a form with more than 6 pages, you do not need to complete pages 7 thru 9. Please return your completed form signed by a doctor, parent and player as soon as possible to the Tri-Valley High School Office.

Additionally, new this school year is the implementation of the District's Drug Testing Policy. Tri-Valley students participating in athletics require a form signed by the student's custodial parent or legal guardian to be eligible for random testing selection. More information can be found in the policy links below. Please return the completed consent form to the Tri-Valley High School Office.
