Teaching or Substitute Teaching Certificate, issued by the Arizona Department of Education (A.D.E)
Proof of the MMR vaccination IF born after 1956.
If you do not have this immunization record you must have the vaccination redone, a blood test from your doctor to prove immunity, or an immunization waiver.
Contact information (including email address, phone number, and mailing address) for a minimum of 3 professional references, one of whom must be considered a supervisor.
Cannot be family member through blood, adoption, or marriage or a family friend.
Should be acquainted for at least 6-months.
Former supervisors are preferred.

Up to 6 months before expiration or less than one year after expiration.
If you choose to mail your application to Phoenix, it can take over a month to get your certificate.
The Phoenix Office also offers same-day certificates for walk-in customers who wish to drive to Phoenix.
List any employment experience, even if it was more than 4-years ago.
List activities within the past 4-years to establish a "work history." This can include:
Community volunteer hours/service
Religious volunteer hours/service
School classroom, club, or parent volunteer hours/service
Membership/involvement in community, educational, or religious organizations
Full-time or part-time student
Stay-at-home parent
Account for time over the previous 4-years.
Highlight experience(s) that show qualities of a good substitute teaching candidate.
Provide contact information for community members who can vouch for character and work ethic.
Community organization leaders are a great resource for supervisor references.
For school classroom or school related volunteer work, teachers, parents, or community members with whom you have worked.
