The templates below can be used to track data for various treatment protocols.
Target Behavior by the scheduled time (Frequency of target behavior by schedule & Opertunity Based data)
Identifying Enviormental Events
ABC analysis helps clarify challenging behaviors by examining what happens right before (Antecedent), the behavior itself (Behavior), and what happens immediately afterward (Consequence). This straightforward method reveals the environmental triggers and consequences maintaining behaviors, enabling clinicians and educators to adjust settings and responses. By clearly seeing these patterns, effective interventions can be designed to decrease unwanted behaviors and promote positive change.
Understanding What Drives Behavior
Traditional ABC analysis focuses on triggers and consequences, but EO-Behavior-AO analysis takes things deeper, uncovering why behaviors happen. By examining motivational factors—what drives behavior (EO), what the behavior is, and what effectively reduces that motivation (AO)—this approach pinpoints precisely why certain behaviors continue and how to effectively address them. We can then teach meaningful replacement behaviors, directly reducing problematic behaviors by meeting underlying needs more effectively.
This template will graph the following
Up to three problem behaviors
Up to three skills to increase
The graph will also calculate the following data based on the data input for each challenging behavior and skill increase.
Average problem behaviors during the last 10 days
Days with 0 instances of the problem behavior
Increase or decrease in problem behavior from baseline.
For skill increases the average percent correct for the last 10 days entered and the change from baseline.
Phase Change Lines and Phase Change Labels
Creating a phase change line can be done by inserting two rows in the data set where you would like the line to go. Then you put a date that is in between the two dates where the rows were inserted. Finally, you remove any points from that series to make a straight line.
To make phase change labels you can format a custom data label to create any text that you need.
If you have more than one phase change line the data will need to go into it's own column.
This template allows you to measure goals based on schoolwide expectations.
The template allows for 4 goals based on expectations
Make a copy of the KCUSD SBT data sheet and place it in the student's file on the Psych/ Behavior Team shared drive.
There is a setup page to enter goals and student information. Fill this out completely before modifying any of the other sheets.
A Printable point sheet is generated after the setup sheet is completed.
A progress sheet will show a line graph for the progress on each goal as well as total daily points.
All data is input on individual worksheet tabs. The tabs will need to be modified to match the title of the expectation being measured.
Use the setup sheet to input all information from the Student's Behavior Plan.
