Westfield Washington Schools is committed to working with our employees to
provide a safe work environment.
It is our policy that employees should report unsafe actions and conditions to their supervisors. We all share the responsibility for the overall health and safety of district employees. It’s important that all employees follow safety rules and are aware of their surroundings.
Workers’ compensation insurance covers all employees in the district. If an employee is injured on the job, they shall immediately notify their supervisor, school nurse, or building administrator. If the injury occurs on a school bus, the bus driver shall notify the transportation director. Timely reporting of the incident will assist us in ensuring that proper medical care is received and your claim is promptly reported to the workers' compensation insurer for Westfield Washington Schools.
For work related accidents / injuries, Indiana Code 22-3-3-4 grants the employer or their workers' compensation insurer the right to direct medical care including selecting the care provider. However, in an emergency, the injured employee will be sent to the nearest treatment facility. Follow-up treatment will be directed by the workers' compensation insurer.
An employee should only go to the Emergency Room in the case of an extreme medical emergency. The Riverview Employer Health Clinic is NOT to be used for work-related injuries; employees are required to utilize WorkMed for non-emergent injuries. Use of unauthorized providers, including your own physician, may result in charges not covered by workers' compensation.

Notify the school nurse and administrator in the event of the injury at the time of occurrence. They will guide your next steps. If medical treatment is required, the employee shall only use approved providers. Most care will be coordinated through Riverview WorkMed Occupational Health Services.

The nurse or administrator will fill out the Indiana Workers’ Compensation First Report of Employee Injury, Illness Form (State Form 34401) and the employee will complete the Medical Authorization Release Form the day the injury occurs. These forms can be obtained in the clinic or in the front office of all buildings. Both completed forms will be emailed to Nicole Holstein.
A copy of the First Report of Injury, Illness Form will be provided to the employee to take to WorkMed. This serves as proof of employment by Westfield Washington Schools and is required for treatment.
It is the responsibility of the employee to forward all injury care reports and receipts to Nicole Holstein no later than the day after care is provided.
It is the responsibility of the employee to immediately inform their supervisor of any restrictions to their regular work duties. The employee will communicate regularly with their supervisor regarding medical status, particularly in regards to job limitations and a return to work plan.
If an employee receives care for a work-related injury from an unauthorized provider without prior approval, they may be responsible for resulting charges.
The employee is expected to comply with treatment recommendations of the practitioner.
