Paperwork
Fill out the forms in Google Classroom. Parents/Guardians will need to read and digitally sign each form. There are multiple locations for signatures. This information needs to be filled out only if the student made the team.
Athletes will NOT be able to participate until all forms are completed. Forms are available through the team's Google Classroom. The code to the Google Classroom will be given to those athletes who made the team.
Athletes are also required to turn in a copy of their physical form filled out by their doctor to Coach Schreck a week before the first game (physical forms are good for 1 year).
Athletes are required to pay a $65 dollar district fee and a $10 bus fee before the season starts. An additional $25 spirit pack fee is also required for a total fee of $100. Fees are payable online and in the school's office. Receipts must be turned in to Coach Schreck. If the student qualifies for fee waiver, the office will provide a waiver voucher to be turned in to Coach Schreck.
