Life happens and the District wants to be there for you when it does. That is why the District partners with Madison National Life Insurance Company to help employees plan for the unexpected.
Employees who work at least 20 or more hours per week.
The district offers two different life insurance options:
100% District Paid Life Insurance
No cost to the employee and automatically enrolled
This life insurance pays a 1x an employee’s annual salary to their beneficiaries if they pass away
Supplemental Life Insurance
Voluntary and 100% Employee Paid
This is extra life insurance for employees and their spouses
It is age rated and based on the employee’s age, even for their spouse
Employees can get a maximum amount is $200,000
A spouse can get a maximum amount of $150,000
Employee must take supplemental life for themselves if they are taking it for a spouse and a spouse cannot get a higher amount than the employee has for themselves
The district offers two different Disability insurance options:
100% District Paid Long Term Disability (LTD)
No cost to the employee and automatically enrolled
This disability benefit replaces income lost by a covered employee when he/she cannot work due to a disability
There is a 90 day elimination period for benefits
On the 91st day it covers and pays 90% of the employee’s base salary for 2 years. After that you must be deemed permanently disabled by ins company
Short Term Disability examples
Voluntary and 100% Employee Paid
This disability benefit is designed to preserve income during the early months of a disability
The benefit payable cannot exceed 66 ⅔ of your annual salary divided by 52 (weekly salary)
Short Term disability payments can be used in conjunction with illness time, etc
You can enroll at any point throughout the year. However, if the enrollment is not within your 30 day new hire period, you will have to complete an evidence of insurability (EOI) form for the insurance company. They will use this questionnaire to approve or deny your enrollment.
If you are enrolling outside of your 30 day new hire period, you will need to complete the two steps below:
Complete the enrollment form, linked here, based on what insurance you are enrolling in and send it to the Human Resources Department.
Complete the evidence of insurability form, linked here, and send it to the insurance company’s address listed on the form.
After a while, you will be notified by the insurance company via mail if you were approved or denied. If approved, the premium will then be taken from your paycheck starting the approval effective date.
Please note: If you are enrolling a spouse for voluntary coverage outside of your 30 day new hire period, you will need to complete the enrollment form and they will need to complete the evidence of insurability form to send to the insurance company.

For Short Term and Long Term disability, you will need to complete both the employee statement and have your provider complete the physician statement and fax both back to Madison National Life.
If you need to update your beneficiary designation, please click here for the form. Once you update it, please return it to the Human Resources Department.
