It is recommended that you include information about the student in each of the following areas (headings are not required but are helpful):
Background Information: Grade, school(s) attended, identified disability, family, any secondary/additional disabilities, etc.
Child's Strengths/Weaknesses: Just a summary; can include such things as does/doesn't ask for help, does/doesn't complete homework, does/doesn't participate in class; academic summary comes from ETR, IEP goals, current progress, progress monitoring of IEP goals
Parent Concerns: can state...Parent's do not have any concerns at this time
Child's Interests: ask the student
Relevant Medical/Safety Info: this should be items not included in the PLOP because they are NOT related to the goals/objectives
Interventions: those that work/don't work
Accommodations: specific accommodations will be listed in other sections, but a brief statement or summary can be included here
Information about special factors: (Limited English Proficient, Communication, Behaviors, etc.) If any were marked "YES" above, then include a statement about it here
Summary of Related Services: summary from any/all related services

Needs from ETR not addressed in IEP: This includes any areas of need that are identified in the ETR but are not being addressed in the current IEP. Perhaps the student no longer has needs in a particular area (if the ETR is more than a year old, this could happen), or if there are a lot of areas of need identified, it may not be possible to address them all within the IEP year.
Statewide/Districtwide Assessments or recent ETR testing: Results from most recent assessments with accommodations or alternate assessment; If exempted from consequences of TGRG and why; HS students - it's helpful to include end of course exam results, current courses and grades, as well as credits/on track for graduation or needing credit recovery.
Post-Secondary: (ages 14 and up) if NO concerns about living independently, write a statement here and then you do not have to write a goal and activity in Transition Plan/Section 5
Preschool: include a summary of the child's developmental strengths and opportunities for growth in the areas of adaptive behavior, cognition, communication, hearing, vision, sensory and motor functioning, social emotional skills and behaviors as well as pre-academic skills
