PA School Health Law requires a medical examination for students in Kindergarten and 6th grade. Examinations may be performed by your primary care provider. Please return the completed physical forms along with an updated immunization record to the school nurse. If you are unable to have the examination completed by your provider, the exam may be performed by the school doctor at no expense to the family.

Important note- the first meningococcal conjugate vaccine (MCV) and Tetanus, Diptheria, Pertussis (TDap) booster is required for all 6th-grade students. Proof of this immunization must be provided to the school nurse prior to the first day of the student’s 7th grade year. The 6th-grade physical is an ideal time to have these vaccinations administered. (Vaccines are NOT administered when physicals are performed by the school doctor).
