PART 1: Basic Demographic Information & Short Questionnaire
Applicants who have not previously served as a Teen Counselor at Jamestown 4-H Center’s 4-H Junior Camp.
Applicants who have served in prior camp seasons as a Teen Counselor at Jamestown 4-H Center’s 4-H Junior Camp.
Applicants are expected to submit:
A resume highlighting leadership, service, school involvement, or extracurricular activities (note that “work experience” is not required unless useful for your application)
A cover letter explaining:
Why you want to be a teen counselor
What strengths you bring to camp
What you hope to gain from the experience
These documents help demonstrate readiness, responsibility, and professionalism and are aligned with the 4-H Beyond Ready framework that we are adopting to make our members more prepared for future opportunities (i.e. in the workforce, for college, military, etc.)

PART 3: References
Each teen, new or returning, must have two references completed by non-family members. Teen applicants are responsible for sharing the link and identifying which reference form you should complete.
