We had a unique situation when interviewing autistic youth and found that creating a comforting environment helped the youth to feel better about answering the questions if they felt connected to a near peer.

It is in this form that the IRB reviews the questions, not in the form of the slides. Also when asking for permission for Visual Design Rapid Prototyping, say you will be making changes to visual design based on feedback and accessibility in user testing and play testing.

Sometimes we include probing or follow up questions that are predetermined.

Then research observers watch for verbal and non verbal behaviors that can't be captured in recording and note those. Then the observations are summarized and grouped in like categories to start to tell the story of the user experience.

You should do the survey, interview or focus group based on the product you are analyzing and have your questions lead you to insights for making that product better. You can show them the product, or talk about it's goals and get feedback.
Note: If you are going through the IRB for approval, each question and method of questioning (individual or group, survey or interviews) will need to be approved.
Decide which technique for finding out about what your users want based on your project.
Choose the technique, will you do interviews, surveys and or focus groups. Your choice.
Create your questions.
Make a form that is easy to fill out when working with your users. (Google Forms or Survey Monkey work well, you may need to use qualtrex if you have a high number and want to have an organized system of collecting data and instant replies that is typically approved by MSU IRB.
Schedule your users.
Get commitments. - this takes time! Plan ahead.
Work with your diverse group to ask your questions. Consider socio-economic status, gender, race, ability, age where appropriate for the audience.
Find the focus that they tell you about your content.
Perform the interview/survey/focus group.
Analyze your data. - You will learn more about this in this module. (And the interviews and the data analyzation typically can't be done in one week -so save some time for these steps.)
Write a summary. This should include how your goals may have shifted after hearing from your users. This should include body language, big goals that they hoped for towards your project.
Add this content to your presentation. (there are no set slide templates for this in the current presentation - just make your own.)
This can be a list added to your slideshow.
Feature lists may include something like this: (this is from a lot of different projects and yours will be unique, it should inform the making of your product.) After you make this list - prioritize them in order.
