TLC has four supportive housing programs. Transitioning to adulthood is hard for anyone, but can be especially challenging for our most vulnerable current or former foster youth as well as youth experiencing homelessness. TLC provides comprehensive mobile case management including employment, health and wellness, education, finance management, accessing resources, and navigating the ins-and-outs of independently living. We help transition age youth (ages 16-25) to rise up from foster care or living unsheltered successfully by providing a safe housing environment where they can learn and practice the life skills necessary to work on self-identified life goals while living independently.
TLC partners with landlords and homeowners to provide safe apartments, granny units, room rentals, and homes throughout Sonoma County.
