Assessment and Care Plans

Introduction

Module VI provides information on comprehensive assessments as these are the basis for the recipient’s service, support, and care plan, as well as the CNAs role in the assessment/observation process.

Evidence-based best practice information regarding plans will also be discussed, highlighting the use of the "I" care plan in the skilled nursing settings. Assisted living and home care providers may want to review the "I" care plan and use those portions that help support person-directed care.

Objectives

  • Describe rules and regulations regarding assessments.
  • Describe the differences between initial assessment and focused assessment.
  • Describe the CNA's role in completing assessments.
  • Discuss the roles of individuals input into the care plan, keeping in mind you are all part of the team.
  • Describe the process of the "I" care plan.


Note: For the sake of simplicity, in this module, we will use the acronym CNA to include caregivers such as CNAs in nursing homes, care managers in assisted living, home health aides, and other direct care workers. Participants can apply the objectives, information, and tools that are most appropriate to their care setting.
