Confirmation Letter of Eligibility

A Letter of Eligibility from the Sponsor’s home parish, dated within a year of the Confirmation sacrament date is required (including sponsors from St. Maximilian Kolbe Parish).

The Pastor of the home parish is responsible for confirming that the sponsor meets all requirements pursuant to Canon Law.  

Have your student’s sponsor contact their parish Rectory to identify the specific process for acquiring this document.  

The Letter of Eligibility should be submitted to the St. Maximilian Kolbe PREP office with the Confirmation Registration Form, unless mailed directly from the Sponsor’s home parish to the PREP office.
