1. Candidates Baptismal Certificate, if not Baptized at Saint Maximilian Kolbe

2. Sponsor Letter of Eligibility (Sponsor must obtain this by calling their parish.)

3.  $100 sacrament fee (Please make payable to Saint Maximilian Kolbe PREP Office) (This is only for Saint Max School Families, PREP families prepaid through Parish Giving)
