If you are having problems logging into netfocus to complete the evaluation online, try these steps:
Close entire browser window (I use chrome as it seems to work best)
If you get a warning about the certificates, you can click advanced and continue to the webpage, these may pop up 2-3 times as you progress through the website.
If you still have issues use the PDF version of the form and send it back via email to stephanie.varnell@navy.mil. Ensure all fields are completed as it must be inputted into the system and will not save if there is any missing information.

Once you have completed the evaluation either online or using the below form, email a signed transcript and a copy of the school profile to stephanie.varnell@navy.mil
The transcript does not need to have a school seal if it is signed and dated by a school official
There is a very short timeout on the system. I recommend looking at the PDF below to gather the information ahead of time. If you would like to send a letter of recommendation in addition to this form, you can email that to me as well.
Math/Science Teacher

Math & Science Teachers can submit the evaluation online using the emailed login and password. There is a very short timeout period and I recommend reviewing the below form and typing your responses and scores in a word document to prevent any rework, then once logged in, cut and paste it into the system. If you get an error after submitting, you can email your word document with scores and I can enter into the system for you.
Other Teacher, Employer or Coach
Other Teacher, Employer or Coach
Coaches, Other Teachers and Employers can submit the evaluation online using the emailed login and password. There is a very short timeout period and I recommend reviewing the below form and typing your responses and scores in a word document to prevent any rework, then once logged in, cut and paste it into the system. If you get an error after submitting, you can email your word document with scores and I can enter into the system for you.
