Person Centered Planning gives students the opportunity to learn about themselves and their preferences and decided what they want to share and the steps they want to take towards achieving their goal. The above person centered planning tool allowed individuals to create a visual representation of their decision making and plans for the future.

As a trained Transition Specialist, I have demonstrated the ability to create a research based transition planning assessment to evaluate the strengths of a transition program. I can consult with program leaders to help them identify the strengths and needs within their programs and suggestion how they can improve their programs.
As a Transition Specialist, it is important to me that students learn their strengths, needs, interests, goals, and members if their support systems. It is beneficial for students to lead their own IEP meetings so that they are able to present themselves in a way which increases the likelihood of their success in post-secondary environments.
