I have developed an interagency team which meets monthly. This is effective to ensuring that all team members are meeting the needs of the collective goal for each of our students. Also, we are able to encourage, educate, and share information and resources which we can all benefit from. This is essential to ensure that we are doing what is best for the students we serve.
As the Transition Specialist, I organize meeting dates and times, and invite new members to the team. I am a liaison between the team members, the school, families, and our students.
My Special Education Director does not always attend our meetings. However, she ensures we have input on her expectations, concerns, and any new expectations from the South Carolina State Department.
Our Secondary Coordinator, she and I work together to meet the needs of our students. She meets with teachers to help ensure IEPs are compliant, structure the classroom setting, and she and I coordinate on how to achieve transition goals and expectations set by the state and our director.
Our Elementary Coordinator has been invited to our Interagency team to help us ensure that our students needs are considered with programming k-12.
Our Department of Disabilities and Special Needs Coordinator does not hesitate to let us know that receiving benefits can take a while. So, they attend meetings, events, and initiatives to inform parents and teachers of their services in an effort to meet our students needs as soon as possible.
Our Vocational Rehabilitation Counselor comes to the high school to meet with students weekly. While she and I meet weekly, sometimes meeting students together, we ensure that our information about students are uniform. She is a great support with helping us to achieve the goals we have set for our students post-secondary planning.
Accessibility is an agency we work with that provides classes and work experiences for our students. They have a lot of information and resources to share about students.
Dorchester Department of Mental Health- there is a mental health counselor at our school who knows our students and some of the struggles that may be affecting our students, but also students in this area who may not be a DMH client.
Dorchester County Career School Counselor - the Dorchester County Career Center is a shared career facility between our district and the other Dorchester County District within our state. However, the career counselor is an administrator, guidance counselor, and a parent to a student that was served through our program.
Parent- We have chosen a parent that is an active participant for child to give us insight on a parent's view of the work we are doing.
We will work towards increasing the percentage of students meeting the state requirement as college and career ready graduate from 75% to 80% in the 2024-25 school year.
Complete a program at the career school, earn 360 hours in work based learning, or make a level 3 or higher on the career- ready assessment.
Maintaining an 80% rate of students making seamless transition (last year 75%).
Increasing parent engagement in activities in and beyond the classroom.
