List of Admission/Enrollment Requirements

Requirements to be submitted:

• Downloaded and duly signed: (1) Application for Admission, (2) Pre-registration Form and (3) Notarized Undertaking form

• Original Transcript of Records (TOR) from the last school attended. (TOR with remarks “Copy for PUP Open University System” shall be submitted within the semester)

• Original PSA/NSO Birth Certificate

• Original PSA/NSO Marriage Certificate (for the married female applicant who opts to use their husband surname)

• Certificate of Employment

• Filled-up Recommendation for Admission

• NBI/Police Clearance within the last 6 months (if unemployed)

• Updated Curriculum Vitae/Resume (including research works and publication, if any)

• A two-page double-spaced write up/concept paper discussing or describing the research area, idea, or interest that the applicant would like to investigate in his/her thesis. The write-up should be on short bond paper and encoded using Arial font size 12.

• Two copies ID pictures, 2x2 inch (recent, identical, in formal attire, white background)
