This involves a retrospective review of a quality improvement project that a pharmacist has completed.
Any audit or service evaluation project could be used. Although re-audits are acceptable, pharmacists will learn more if they are given the opportunity to identify a project's objectives and design the method themselves. The project may involve evaluating a quality improvement initiative identified as part of an action plan following a previous audit, patient satisfaction survey or error / services of errors that have occurred in the organisation, e.g.
Implementation of a new medicines reconciliation form in response to an audit identifying that the quality of documentation was poor
Implementation of a new LMWH prescribing pathway on the electronic prescribing system in response to a series or dosing errors reported
The pharmacist is allocated a quality improvement project and set a deadline for completion. The completed project is reviewed by a senior colleague who provides feedback on the strengths and areas development demonstrated. There may be a report and / or project presentation to review and the pharmacist and assessor should discuss the project to assess the approach taken and knowledge gained by completing the exercise.
