Pre-departure information

You will need to log into your MyStudyAbroad portal and complete all required forms before departure. All forms can be uploaded online directly through the Portal. 

• Submit the post-decision questionnaires: 

  1. 1. Emergency Contact, 

     2. EU GDPR Acknowledgment & Consent, 

     3. Name, Email and Photograph Release

• Read and electronically sign the post-decision materials: 

  1. 1. Host University Application, 

     2. Authorization to Receive Host Institution Transcript, 

     3. Billing/Withdrawal/Refund/Cancellation/Dismissal Policies,

     4. Course Approval Instructions and Requirements 

     5. Program Dismissal Form

     6. Study Abroad Health Assessment

     7. Study Abroad Placeholder Course Registration

     8. Well-Being Abroad

     9. About International Safety & Security

     10. Safety Abroad

     11. About International Insurance

     12. Using Your International Insurance

• Complete the health and safety quiz

Your Study Abroad advisor will provide instructions on the Photocopy of Visa, Travel Itinerary, Universitaly pre-enrollment summary, and the University of Pavia Acceptance Letter during your Pre-Departure Orientation Meeting. Keep an eye on your inbox!
