Eligibility: Active NFL Alumni Paid Membership (additional eligibility below) Dentists in your state have volunteered to provide dental care.

Service: Free dental care to eligible applicants. Applicants must meet one or more of the following:
1. Have a permanent disability
2. Are over the age of 65
3. Are medically compromised (includes signs and symptoms of memory loss, joint replacement, and spinal issues)
