APS = Adult Protective Services (It used to be called VAPS: Vulnerable Adult Protective Services) is funded through a separate program. Their billing (Kaitlyn, Kayla, and Courtney) needs to be submitted monthly to Scott Hague. If you ever need help, he's very good at explaining things and is a very patient helper. :)
The goal is to have this in by the 5th of the month.
Step One
What you need:
Verizon bill
Travel sheets from the workers (Kaitlyn, Kayla, and Courtney)
Payroll from Nichole or Randi (HR at the courthouse)

You'll want to use the most recent version (the one that isn't signed) and copy it, change the dates, and save as the current month

From the Travel Sheet
Kayla and Kaitlyn will have personal mileage and meal totals to copy into the worksheet template.
Courtney's mileage will come with her pcard receipts - she uses a zone vehicle so I take the total of the fuel receipts and put that in for her mileage. Her meals come off of her travel like the other two ladies.
From the Payroll Sheet
The salary is not the gross salary - it's the "REG SALAR" line.
FICA, Medicare, PERS, Health, Dental, Vision, and Life come from the right most column
From the Verizon Bill
All three of their phones should be the same unless one of them upgraded their phone. Always check the verizon bill to make sure
If they ever order supplies or go to training, that is also included in this sheet.
Step Three
SFN 1763
On the SFN Form, the numbers from Column C should be transferred over to Column A before anything else is added
In Column B, the numbers from the BOLD sections on the Worksheet Template should be put in the Salaries & Fringe Benefit and Travel boxes on the SFN form
If the numbers from the "Total Expenditures" line and "Minus previously billed" line (from the Worksheet template) match the bottom numbers from Column A and Column C on the SFN form, you've done it right.
Step Four
Make sure the dates are all accurate. The Contract Period dates do not change.
The Billing Period dates change and the Date in the right-hand column changes.
Insert your signature on the "Payee Signature" line and send both the Worksheet Template and Signed SFN form to Scott Hague. shague@nd.gov
Every two years, the contract for APS is reconfigured. I have put all of the documents that I used in the summer of 2023 into the google drive.
