Exam Information

Part 1


If you have already applied to IRCC and have received a form called Medical report client biodata, it will have a bar code and an IME# on it. Please check the papers/email you have received from IRCC to see if there is a bar code and IME#. I need that number to look up your file in the system. Sponsored clients always have an IME#.

Refugees have a UCI#/client ID# on their refugee claimant form. We need to search for your file in the system using these numbers.

If there is no number (UPFRONT EXAM)we can create a file and give you a number.

ON THE DAY OF YOUR EXAMINATION

Please bring the following items to your medical examination appointment:

  1. Identification - preferably Passport (but will also accept a certified copy of passport or Canadian drivers license for Canadian exams only). Refugee ID or acknowledgement of claim form is accepted for Refugee's.

3. Glasses if you wear them. We check your vision and we need to record the best vision you have

4. Any Medication that is taken on a regular basis - we will need to know the name of the medication, the dose and frequency. It is best to write these down or bring them with you.

5. Any reports related to relevant past medical history in particular with a diagnosis of cancer or psychiatric illness.

6. Bring an interpreter if needed.


It is no longer necessary to bring photos as we take your photo now and upload it to you file.

Drink lots of water in the half hour prior to the exam as we do a urine test on all who are 5 years of age and older. (So it is especially important for children to drink water.)

You do not need to fast, in fact we prefer that you eat and drink normally.


URINE TEST

During the exam we perform a urine test to check for blood, protein and glucose (sugar). Females please take note and try and schedule the appointment in consideration of your monthly cycle.

THE SAME ID SHOULD BE USED FOR ALL EXAMS/TESTS/APPOINTMENTS.

IRCC specifies what we can accept as an identity document. We prefer valid passport or certified copy of passport. Drivers license is also acceptable. Refugee protection form is accepted for refugees. No other forms are acceptable.

For New Zealand exams only passport is accepted as id


UPON LEAVING THE OFFICE:

Upon leaving our office you will be given the following:

1. a receipt for payment(not refugees).

2. the x-ray requisition for all 11 years of age and older

3. The Lab requisitions for blood tests for all 15 years of age and older

4. a page with a bar code and file/health case number on it.

Keep this page safe. You may make copies of it. You may use this page as proof of having done the exam.

Please check all the data on the page and ensure all the information is correct prior to leaving the office. Also please ensure that you get your passport back.

Take the page with the bar code with you to the x-ray department and they will look up your file in the system using the number, however get the page back before you leave. The radiologist will later upload the x-ray film and report to your file. You can give a copy of the paper to your lawyer if you are using one. You can send a copy to your immigration office if they request verification of the exam.
