COVID-19 Client Information

As we continue to monitor the Coronavirus Pandemic, we are following recommendations from the Ontario College of Family Physicians. For the safety of our staff and other clients, all clients will need to be screened prior to attending an immigration medical examination through a mandatory pre-screening questionnaire. Please click on the questionnaire to the right to download and complete the questionnaire and send a copy to Dr. McDonagh at mcdonaghimmigrationservices@gmail.com. We ask that all clients complete the questionnaire with honesty to ensure the safety of others. If you do not understand the questions, please fill out the questionnaire with the help of an interpreter.

We need a COVID-19 questionnaire completed for each family member who will be attending the clinic for a medical examination.
