Open SAAnesthesia > close the case selection window
CTRL + ALT + SHIFT + right click (all at the same time) with your mouse positioned next to Help
Select Disassociate
A list of currently unfinalised records will appear (including records currently in use by other users)
Find the record that you need to disassociate ensuring that the Date/Time and Created By is correct (please note, records from other sites will also display here)
Select the record
Select Disassociate
Record will disappear from the list
Since the record has been disassociated from the scheduled case, it can now be found as a Blank Record - select this option
1. Using the Date, Time and Created By information, select the correct blank record
2. Click OK
The record will then open with a blank demographics bar (as it is now a blank record)
Select Associate Case to Record
Select the correct scheduled case (double-check it is the right patient!)
Select OK
The demographics bar will update with the selected patient’s data and the record will now be associated to the correct case.
IF you cannot resolve the issue by disassociating - re-associating the record, please do the following:
Transcribe the data onto a paper anaesthetic chart labelled with the correct patient details and ensure that this is filed in the patient's pink scanning folder
Document an Anaesthetic Note on PowerChart indicating that a paper anaesthetic chart exists with basic pertinent information (e.g. type of anaesthetic, complications, etc). Please include any information about the conduct of anaesthesia that would be useful for a future anaesthetist (in case the paper anaesthetic chart is not scanned onto Mediviewer in a timely fashion or gets lost!).
Ensure that key intraoperative drugs are prescribed and signed for on the patient's Cerner Drug Chart:
Paracetamol
Diclofenac
Antiemetics
Antibiotics
Blood products
Please use clinical judgment for drugs not on this list.
4. Delete all information from the Cerner Anaesthesia Record from the incorrectly assigned patient (actions, vital signs, drugs, fluids, etc)
5. Enter a freetext note on the Cerner Anaesthesia Record to explain why the data has been removed
6. Inform the anaesthetist who may be impacted by this error (i.e. If the incorrectly assigned patient is from another theatre's list)
7. Complete a DATIX assigning Dr Moosajee as the handler
