Behavior Health (BH) and Physical R patients, can be identified by reviewing the contact notes in each
patient’s information (See image below as an example)
Unlike other scenarios when a patient desires to reschedule their BH or PR scenario
Steps For Contact Request
• Click on the admin tab.
• Press the blue +
• Assign to Individual or Group: Select group on the drop down.
• Assign to Individual: Enter the clinic based off where the pt is seen. Ex: “Victorville Clinic”
• Enter your notes. (Make sure it is descriptive so that once it hits the clinic they know where to
route the contact request)
• Once notes are completed, press “Add”.
Notate In Activity Details
Always following a template, fill in the notes, add any additional information needed. Select a
disposition.
Select Contact Status
Choose the contact status “Will Not Attend (by phone)”
Additional Notes:
If patient, ask what a B.H. one on one appt is:
“This appointment is a one‐on‐one behavioral health session, meaning you’ll meet privately with your
provider to focus on your personal mental health needs, goals, and care plan. It is separate from a
group session.
PR (or Therapy) patients can be also easily identified by the blue check mark under “Therapy”
We only called patients for Therapy reminder 7 days before, we do not call them the day before.
