Option 1-- If you can bring your guardian to our Thursday October 14th meeting at the Wheat Ridge Rec Center between 4 and 5:30 PM: (Note, we will stay until 6:00 PM if any parents want to arrive later.) We will have print out-- you will sign, print and notarize there. Note: the notary has to actually watch you and your guardian sign the forms-- so, you cannot bring them "presigned"! Please bring an ID and a mask.
Option 2-- If you cannot bring your parent or guardian to the meeting: Print out all 5 forms below. With your guardian, visit a notary to sign the agreement-- which is the first form. (This could incur a fee.) Also have both you and your guardian sign the remaining forms. Bring all 5 of these signed forms with you to the October 14th meeting.
Option 3-- Mail or drop off in Lakewood: Print out the entirety of all 5 documents below. Take the first one, the Letter of agreement, to a notary and, with your guardian, sign and notarize. Then sign and date the rest of the forms. Double check that you have signed everything correctly. Then either mail (posted no later than Tuesday, 10-19-21) or drop off (no later than Wednesday, 10-20-21) to the following address:

All eligible individuals are strongly encouraged to get vaccinated.
With the recent passage of Public Health Order 21-005, interns may be required to be regularly tested if they are not fully vaccinated in order to participate in in-person events. Please see also FAQs about this public health order. This is all the information Jeffco CTC staff has been provided at this time. We are actively seeking clarification about how or if this will be implemented in relation to this program.
