Members who will be needing accommodations to return to work may request a 504 plan from Personnel. Members should bring their job description to their physician and collect the following information in a formal medical report, not a handwritten note on a prescription pad, that sets forth:
1. an assertion that the employee requires a workplace accommodation because the employee suffers from a medical condition;
2. outlines the nature of the individual’s disability;
3. how the individual’s disability impairs one or more of the individual’s major life activities or daily functions;
4. the letter must also explain that the doctor understands the employee’s essential job functions (teacher/secretary/paraprofessional, etc.); and
5. a list of accommodations the doctor recommends and why the accommodations are necessary to allow the individual to perform their job functions. This will be specific evidence of your disability and medical history.
Gather the above materials and contact Dr. Jennifer Sharp in Personnel. If you would like association guidance, please contact Meg Santonacita at msantonacita@frhsd.com or Sarah Reichenbecher at freaunion@gmail.com to set up a call.
***District paperwork will request that you give permission for your physician to write a summary narrative of your condition. This IS NOT access to your entire medical records. You do have the right to refuse this, but the district also has the right to request you are examined by a doctor of their choosing in relation to you disability at any time. It is important that you revoke this once your 504 plan is no longer needed.
A qualifying disability is any physical or mental impairment that substantially limits one or more major life activities.
