To update status in SSS please fill out the E4 Form downloadable here: https://www.sss.gov.ph/sss/DownloadContent?fileName=SSSForms_Change_Request.pdf
Download, Print then fill out the form;
Indicate the updates needed
Attachments:
For Single - Married: Original PSA copy of Marriage Certificate or Certified True Copy Civil registered Marriage Certificate
For adding dependent: Original PSA copy of Child/ren's Birth Certificate or Certified True Copy Civil registered Child/ren's Birth Certificate
Once done completing the form, please email the soft copy to 247phes.optus.@247.ai for checking. Once ES validated documents are complete, please forward the hard copies to [24]7 Megamall site addressed to Amy Glenn Perez/ Employee Services.
Philhealth
Fill out the PMRF downloadable here: https://www.philhealth.gov.ph/downloads/membership/pmrf_012020.pdf
Download then fill out the form, indicate the updates needed. Since they now accept soft copy, you may edit the form using any PDF editor. Just make sure you insert your e-signature as well.
