1. Enter your Pag-IBIG Membership ID (MID) Number, and active Philippine mobile number to receive the One-Time-Pin (OTP);
2. Type in your Cash Card details; and
3. Upload photos or scanned copies of
• Front and reverse pages of your application form signed by your company’s authorized signatory (if employed),
• One (1) of your valid IDs and
• Selfie photo while holding your cashcard and valid ID.
ES Team shall email you a pre-signed copy of your Pagi ibig Loan form. Please log your request here.
This is for employees who can't file via Virtual Pag Ibig. Pag Ibig requires original copies of the form with HR signature.

Forms will be endorsed to 2F [24]7 Plaza lobby by 5:00PM of Friday.
For those who opt to route the form to other sites (Mega/CS). This will be routed to your site by Monday/ by schedule depending on the number of requests.
Pag Ibig Buendia branch personnel shall drop by [24]7 Plaza site to pick forms for processing. Pick up of forms depends on number of forms received, should have at least 2 or more.

You may get a blank form at 2F ES pod inside red 4 layer drawer beside the door. (Named as HDMF Loan form / Pag Ibig Calamity Loan form)
Please completely fill out the form then attach a copy of your latest 1 month payslip
Attach as well a photocopy of your 2 valid IDs with 3 specimen signature
