Please ensure that PART III of the form is accomplished by Attending Physician.
Attachments:
Home Confinement: Original or CTC (Certified True Copy) of Medical Certificate, Laboratory, X-ray, ECG and other diagnostic results; Surgical/Operating Room Record (if with Outpatient Procedure)
Hospital Confinement: Original or CTC (Certified True Copy) of Operating Room Record, Medical Abstract/Discharge Summary, Medical Certificate, Lab tests and results, Special Diagnostic Procedures, etc.
Photocopy of SSS Digitized ID (front & back) OR 2 Valid IDs (front & back) with 3 specimen signatures
Notification Requirements:
To avoid declined applications, please ensure that Employee Services Team will be notified by submitting your accomplished SSS Sickness notification form via email (247phes.optus@247.ai) following the timelines below:
Home Confinement: within 5 calendar days from the start of sickness or injury;
Hospital Confinement: within 1 year from the start of confinement in the hospital
Note: Once all documents are completed, please send scanned copies of your form together with the attachments to 247phes.optus@247.ai for checking. Once Employee Services Team confirmed that documents are all good, please forward the original copies to [24]7 Megamall site addressed to Amy Glenn Perez or Employee Services Team.
