So you decided to apply for emergency medicine residency... amazing! Welcome to the tribe!  

If you rotate with us and would like a SLOE please email BOTH clerkship directors with the following: 

1. Official ResidencyCAS SLOE request form 

2. Photo

3. CV

4. Unofficial (or official) transcript

5. Board Scores 

Remember to entitle your SLOE request email with your name and SLOE request so that it is easily searchable and accessable for the clerkship directors. (example: Jalen Hurts SLOE request)

We expect your SLOE materials to be sent to us by Sept 1 (Unless you are on rotation in September). This will allow us enough time to write your letters.
