Attendance Sheet

Please note that you will need to have an attending sign your shift log to indicate your attendance.  You MUST have an attending sign for each shift to confirm your attendance!  Not submitting this log could prevent you from passing the rotation!!

You will need to email your attendance sheet and your case write  up to BOTH Dr. Ng and Dr. Crossman  at the end of the rotation to receive a grade for this rotation.
