When ’Personal Record’ button in the Main Page touched this ‘Personal Record’ page is viewed which directs into Medical Details (Self entered medical history by the consumer), Medication details (self-entered), personally maintained records such as uploads, links and online notes.

Medical Details page facilitates entry/edit/maintain of their own health related details/history (Medical, Surgical, Allergies/Alerts, Dietary, Obstetrics, Development of children).

Consumer can make these data visible to their providers which will be important for the providers to validate their records.

Further it is possible to view the Provider side history details by the consumer (if the provider has been given access) and make comments to those if there are concerns. 

Entering medication related formation such what medication consumer on in which doses, route, frequency and other related information.

In default this information is visible to the care providers; and consumer can switch on/off sharing. 

This facilitating keeping personal health details in the app and sharing that with the consumers.

This allows keeping text data, pictures, links, and uploaded files.

Validating care provider entered Medical history information such as Past Medical Details, Medication Details, Allergies & Alerts, Immunisation, Family Medical History, Social Details, etc.

If there are issues/missing data in the provider entered information, the consumer can make comments which can be seen by the Provider and update their records as below here.
