Anytime the police/fire/ems are involved. Even if it has little to do with us. (A person had a medical issue in the parking lot and called 911. – if there are emergency vehicles here, just write a brief note even if it says “we don’t know why. Seemed to be someone’s personal health situation.”)
If you think someone might register a formal complaint about whatever the situation is. It is always good to give us a heads-up just in case we get a follow up.
You think it might be an escalating issue
You think/know that this is not the first time something has been an issue/a patron has been talked to about the same problem.
Whatever is going on is just setting off your “Spidey sense” and you think you might need to track down the details later
You think, “hmmmm…. I should probably make sure Julia knows about this developing problem…”
It is just very odd and involves complicated/legal-ish stuff like voting.
At the very least, email your supervisor (or every yourself) with all the details RIGHT THEN. You would be shocked at how little you remember even a couple hours later. Who else was at the desk? What time did it happen? What day? Do you know any name info at all? Brief descriptions. Take the time to write something down so you can communicate clearly about what happened later. That way if you need to fill out an incident report later you will have the details you need.
When you write an incident report:

Save it in the appropriate folder by year
Naming: IR MM-DD-YYYY initial1 initial2 intial3.docx
Fill out what information you have. Ask other staff to add their comments if appropriate.
Give Julia the incident report
Do you have concerns about patron and /or staff safety?
If staff cannot locate a child's guardian (within 5 minutes)
Is something illegal happening?
Not life-threatening, not an emergency
