Connect Care Provider Portal
The Connect Care Provider Portal (CCPP) is a web-based application connecting healthcare providers and facilities using the Connect Care clinical information system (CIS) to external providers, practices and facilities that use other health records. As a bi-directional information bridge, CCPP can improve continuity of care for patients as they move throughout the health system. It allows non-Connect Care clinicians access to information, communications and services provided by the Connect Care CIS.
Use Cases
There are currently three use cases for CCPP:
Via Alberta Netcare Portal (access to Alberta Netcare is required)
For approved External Research Monitors
For contracted Continuing Care and Seniors staff
CCPP via Alberta Netcare Portal (ANP)
With CCPP via ANP, community users can access information about patients who receive care where Connect Care is the record of care, and communicate securely with the healthcare providers who are on their patient’s care team. Accessible information includes:
Discharge summaries and associated encounter notes
Medications, including administered medications and medications prescribed upon discharge
Upcoming appointments scheduled at Connect Care sites
Connect Care Referral status details
Allergies
Test results, such as diagnostic imaging and laboratory results
Eligibility
CCPP via ANP is not broadly available but is in the testing phase with a limited number of clinics. The scope of users eligible to access CCPP via ANP are members of the College of Physicians and Surgeons of Alberta and, in some cases, members of the College of Registered Nurses of Alberta (i.e., Nurse Practitioners). These individuals must have both access to Connect Care as well as an independent Electronic Medical Record (EMR) in the community. These providers can also take accountability for access for their support staff, who can then assist them by using CCPP. Practitioners and practices requesting access must be approved, with considerations including:
A current Privacy Impact Assessment (PIA).
Designation of a lead custodian as primary point of contact for the practice.
Processes for timely review and confirmation of delegated user competence, privacy awareness, training and administrative supports.
