1.If you already have copies of your previous blood test reports, you do not need to undergo another test. However, if you do not have copies, please schedule an appointment as soon as possible with one of the following. Please see the resources below for your reference.

2.Bring your Clinical Preparedness Permit form, immunization record, and any other clinical documents you have. During this visit, your healthcare provider may order a lab test to check your immunity to MMR, Varicella, and Hepatitis B, begin TB screening, or administer the DPT, Polio, Flu and/or COVID-19 vaccine.
💡 REQUIRED BLOOD WORK TESTS
Many students forget the required blood work tests. Please ensure you complete and submit reports for the following:
Measles
Mumps
Rubella
Varicella (Chickenpox)
Hepatitis B
Note: We encourage you to do the laboratory test as soon as possible as the test result may take 2-4 weeks or longer than usual. Delaying the test may result in late clearance review and may jeopardize your clinical participation. We will NOT provide an extension.
If you have completed blood work within the past several years and have copies of the reports, you may submit them for review.
💡 There is no specific requirement regarding how recent your blood work test reports must be. As long as you provide clear and legible copies, they will be accepted.
3.Schedule your N95 Mask Fit Test, as well as your Standard First Aid and Basic Life Support training.
