Step 2: Download the Initial Documentation

New students are required to download the following documents and forms.

1. Clinical Consent Package - The package consists of 4 forms that need to be submitted to Noreen Champagne by a specified deadline. Download the Clinical Consent Package.

2. Clinical Preparedness Permit Form - This is a vital form that needs to be completed by you and your healthcare provider. It is also one of the documents that are required by Concise Health for your clearance review. Download the Clinical Preparedness Permit Form.

3. C-PASS Handbook - This document contains comprehensive information on how to complete your clinical requirements. Download is not available at the moment. C-PASS Handbook 2027-2028 will be available in August 1, 2026.
