The North Carolina Serious Illness Coalition (NC SIC) is a non-profit organization dedicated to improving care for individuals with severe, advanced, or chronic illnesses. They advocate for public policy changes, work to build coalitions, and educate healthcare providers to enhance care coordination and the integration of symptom relief for these patients (Kent et al., 2023).
Sarcopenic obesity is a crucial concern for the NC SIC when it comes to caring for the seriously ill. This condition, characterized by the loss of muscle mass paired with higher body fat, disproportionately impacts the elderly and chronically ill population that the Coalition serves. According to recent studies, over 30% of elderly nursing home residents suffer from sarcopenic obesity (Bouchonville & Villareal, 2013). The related loss of mobility and strength contributes to adverse outcomes in the seriously ill, including social isolation, depression, lower quality of life, higher healthcare costs due to falls and injuries, and increased mortality risk.
To address this pressing issue for their patient population, the NC SIC has undertaken several key initiatives:
Educating providers: Through their workshops and network of clinical partners, the NC SIC spreads awareness regarding the risk factors and symptoms of sarcopenic obesity so providers can better recognize the condition.
Advocating for assessments: The Coalition advocates for state legislation to establish standardized physical function assessments for chronically ill patients over 65. They are modeled after successful programs in other states that systematically identify sarcopenic obesity cases.
Promoting personalized care plans: NC SIC engages with partners to customize nutritional and physical activity care plans tailored to patient's needs and abilities to prevent and mitigate sarcopenic obesity. This includes promoting home-based and palliative rehabilitation models for the seriously ill.
With sarcopenic obesity on the rise nationally and in North Carolina patient populations (Purcell et al., 2021), the NC SIC sees this multi-faceted approach as essential to ensure that those with severe chronic illness can maximize function and quality of life. Their initiatives strive for awareness, assessment, and evidence-based intervention through policy and educational campaigns.
