A number in a colored shield icon is the Quality of the collectible.
A number (or letter) in a colored battery icon is the maximum charge of the collectible.
Numbers (1 - 12) indicate the amount of rooms that have to be cleared before an active item can be used. "0" means that the item does not have to charge.
The letter "R" means the active item recharges without needing to clear rooms.
The letter "X" means the active item can only be used once before disappearing.
The letter "P" means the item is a passive collectible and always provides its effects after pickup.
These icons indicate the primary item pool a collectible belongs to, and match their in-game appearance. This includes chests, as well as the following rooms: angel, boss, challenge, curse, devil, library, planetarium, shop, secret, treasure, and none (the item has unique spawning conditions).
Note: These may not be entirely exclusive, as pools will often draw from one another. For instance, the Boss Rush item pool (after the Mom fight) draws from the Treasure Room pool.
These icons indicate a change in stats, and match their in-game appearance. This includes Damage, Fire Rate, Luck, Range, Shot Speed, and Move Speed.
A green up arrow indicates the stat increases; a red down arrow indicates the stat decreases; a grey right arrow indicates that stat is set to a specific amount.
These icons indicate the item gives Isaac hearts on pickup, and match their in-game appearance. This includes empty or filled red heart containers, eternal hearts, soul hearts, black hearts, rotten hearts, empty or filled bone hearts, and broken hearts.
