Prom 2018--Permission Slips

All students attending Prom 2018, below you will find both the UPHS student Prom 2018 Permission Slip and the Guest Permission Slip.

All UPHS Juniors and Seniors AND their Guests--Please make sure you have submitted your permission Slip

MUST BE SUBMITTED BY THURSDAY, APRIL 26TH

FYI: All PERMISSION SLIPS MUST BE SUBMITTED BEFORE SIGNING UP FOR A TABLE FOR DINNER--NO PERMISSION SLIP NO PICKING A TABLE

Permission Slips have been emailed to all students, are available in the UPHS Office, and are on the Prom 2018 Web Site (SEE BELOW)
