The UPHS PROM 2015--Masked Mystery
Date: Saturday, April 25, 2015
Time: 7:00pm-11:00pm
Location: Visalia Holiday Inn on Airport Drive (Off of HWY 198)
Cost: $55 per person (Includes the dance, a four course dinner, photo booth, saga bag, and other goodies)
Payment: Cash or Checks (Make checks out to the COS Foundation)
Photos: A photographer will be available to take formal pictures starting at 6:00pm
Student Permission Slips and Guest Permission Slips are attached below
PERMISSION SLIPS NEED TO BE COMPLETED AND RETURNED BY APRIL 17TH
