Alberta Health Services (AHS) requires that all healthcare professionals complete a Fit for Work Questionnaire before going to work each day. The questionnaire can be completed up to two hours in advance of your start time.

If you exhibit symptoms of COVID-19:

1. Notify the Cardiology Program Coordinator of your illness.

2. Stay home or go home and enter isolation.

3. Complete the COVID-19 Assessment for Healthcare Workers and Workers in High Risk Settings.

4. If the results of the self-assessment indicate you should take a COVID-19 test, do so and follow all instructions provided to you by AHS and Workplace Health and Safety (WHS).

5. If you test positive for COVID-19 and suspect you became ill while working in the hospital, you are required to notify WHS of your positive test result. 6. Stay home for a minimum of five days from the onset of COVID-19 symptoms, or until your symptoms resolve and you are fever-free for 24 hours (without the use of fever-reducing medications), whichever period is longer. 7. Maintain communication with the Cardiology Program Coordinator (mecardjr@ualberta.ca) during your leave and notify the Program Coordinator when you are able to return to service.
