Clinic Plaques

To recognize your teaching contributions and acknowledge your clinic as a learning site for undergraduate medical learners, the MD Program has a new initiative to provide clinics with signage. These plaques tell patients and others that learners are working and learning in the space and recognize their role in educating new physicians for the community. 

Signs are available in the following sizes:

• Small (9 x 11.5")

• Large (11.5 x 17.5")

To request signs for your clinic, please complete the request form below. 

The request form will be open during the months of September and January. Plaques will be ordered once the request window has closed, and you will be contacted when the package is ready for delivery or pick up.
