Incident Reporting 

RADTH Injury and/or Incident Reporting Procedure 

*In the event of an injury and or incident, please follow the procedure outlined below: 

IMMEDIATELY 

(with blood or body fluid exposure):

1. Remove contaminated gloves, clothes, etc.

2. For sharps injury allow bleeding for 1-2 minutes – do not squeeze. Wash area with soap and water or alcohol based hand rub and cover with a dressing.

3. For mucous membrane exposure (e.g. eyes, nose, mouth) and non-intact skin, flush with large amounts of water for 10-15 minutes.

4. For all other incidents, seek medical attention as needed.

5. Report incident to the Clinical Faculty member or designate, who will then inform the RT Manager.

6. Follow AHS facility specific protocol and call 1-855-450-3619 (Edmonton – ext. 2, Calgary – ext. 4)

REPORTING:

 All incidents must be reported to the Radiation Therapy Office within 48 hours. 

To report visit EHS - Report an Incident and: 

6. Complete the on-line University of Alberta Incident Report within 48 hours.

7. Complete the Alberta WCB Worker’s Report of Injury within 48 hours by following the injury or incident links.

8. For Radiation Therapy Program students, both forms must be submitted within 48 hours to the Radiation Therapy office via email at radth@ualberta.ca .
