There are a variety of different forms and verifications that the Faculty of Nursing Undergraduate Office can complete for you, if required. Please use the following section to determine what steps are required to have these requests completed. If you're unsure if the form is meant to be completed by the Faculty or not, you can send us a ticket or come by the office and we'll be happy to help!

Health Care Aide Clinical Hours Verification: for students who have successfully completed Year 1 and Year 2 courses (Year 1 After Degree).
Undergraduate Nursing Employee (UNE) Clinical Hours Verification: for students who have successfully completed a minimum of 600 clinical hours.
IV Start Certification: for students who have completed the certification exam and obtained 3 supervised IV starts.
Program Completion Letter: for students needing a completion letter for their employer at the end of their degree.
