Leave of Absence Forms

Guidelines for completing the PGME leave form:

·   Please submit leave requests as soon as possible.

·   The Program Director signs/approves the leave form and forwards the form to the PGME office for PGME approval. 

·   The PGME offices ensures  AHS is informed of the leave.

Guidelines for completing the Med Micro Program leave form:

·   Please submit leave requests as soon as possible, preferably before schedules are completed.

·   If already scheduled at an off service placement site (i.e. not scheduled in a microbiology lab),  check with the program/site preceptor for approval before submitting the leave request to the Medical Microbiology Program Director for approval (see Site Preceptor list below). Note that ID does not approve one week leave requests during two week rotations (the two week rotation would be rescheduled to accommodate the leave request).

·   Vacation requests must be submitted within eight (8) weeks of the time-off requested (PARA 20.02c).

·   Program Director has 14 days to approve/deny request (PARA 20.02e).

When scheduled at areas other than microbiology, send forms to the applicable area for approval. Once approved send to micromepc@ualberta.ca
