For those who are unable to generate GIS data when requesting site or document numbers, the OAHP Geospatial CSV templates may be prepared and submitted with a supplementary map.
Prepared CSV files with supplementary maps can be submitted through the online Number Request Form. Please read the supplementary map guidelines and CSV preparation instructions below before submitting your number request.
When using the CSV templates to prepare a site or survey number request, a supplementary map showing the resource boundaries must also be submitted. Maps should adhere to the following guidelines:
All resource(s) must be represented as polygon shapes, not points or lines.
Plotted resources must be identified by the recorder's temporary number.
Maps should have a topographic or street style background.
If multiple resources are plotted on a single map, the map must be high enough quality for a viewer to zoom in on any given resource without excessive blur.
Please note: If you are requesting a document number for a non-survey report such as a research document, historic structure assessment, class I/literature review, etc., the OAHP CSV templates may be prepared and submitted without a supplementary map.
If you are requesting a document number for a non-survey report such as a research document, monitoring report, excavation report, historic structure assessment, class I/literature review, etc., the OAHP CSV "Document_Requests" template may be prepared and submitted without a supplementary map.
Please note the special instructions for filling in the Type, X, Y, and Acres fields below.
Please complete all the fields provided in the template (where appropriate). Some of the fields are provided so that staff can easily assign numbers and import the data into our relational database.
Temp_Num (Temporary Number): Enter the temporary number the recorder assigned to the site, if applicable.
County: Enter the county that the resource resides in. If the resource is in more than one county, list the resource on multiple lines to reflect each county. Each portion of the site in the applicable county will receive a separate site number.
SITE_ (Site Number): Leave this field blank. OAHP will use this field to enter the assigned number.
RES_TYPE (Resource Type): Enter all possible combinations of resource types. If this field is not populated, the resource will receive the highest level of restriction in our database and may not be visible to most people until fully processed. Enter one or a combination of the listed resource types:
Archaeological: Prehistoric sites and isolated finds/features.
Historical Archaeology: Historic sites and isolated finds/features contain buried deposits, features or artifacts.
Historic: Historic built environment sites. Mainly intact historic buildings. If a historic site needs additional protection, please add historical archaeology to your resource type (Ex: Historical Archaeology | Historic).
Paleontological: Paleontological sites – not landforms.
Unknown: If it is unknown whether or not the site is historic or prehistoric. This will be given the highest level of protection because of its unknown status.
Restricted: Site containing burials, Tribal Cultural Property, or other sensitive features that should not be disseminated at the request of a landowner and/or tribe.
RES_Name (Resource Name): Provide the name of the resource if applicable. If a resource has multiple names, separate the names with a semi colon (example: Highway 33 | County Road 2 | Forest Service Road 12E).
Intsct_Res (Intersecting Resources): Does the resource intersect with another resource? Yes or No.
LINEAR (Linear): Is the site a linear? 1 for yes and 0 for no.
Segment: If the site is a linear, is it a segment? Yes or No.
Rcrd_Date (Recording Date): Date the site was recorded.
SOURCE (Source): Name the agency submitting the data.
ZONE (Zone): Enter the Universal Transverse Mercator (UTM) Zone the resource resides in (Zone 12 or 13).
X (X Easting Coordinate): Enter the X UTM coordinate of the center point of the site. Latitude and Longitude coordinates will not be accepted.
Y (Y Northing Coordinate): Enter the Y UTM coordinate of the center point of the site. Latitude and Longitude coordinates will not be accepted.
Acres: Enter the acreage of the recorded resource.
Street_Num (Street Number): Street/house number (built environment only)
Steet_Dir (Street Direction): Street Direction (example: East, West, North, South) (built environment only)
Street_Nam (Street Name): Street Name (built environment only)
City: Name of city where the resource is located (built environment only)
Postcode: zip code (built environment only)
Comments: Please provide any comments that may aid us with your number assignment.
DOC_ (Document Number): Leave this blank, OAHP will assign this number.
County: List all of the counties where field work was conducted.
DOC_Title (Document Title): Title of the document.
Author: List the author’s named on the document.
Num_Sites (Sites): Number of sites recorded.
Num_IFs (Number of Isolated Finds): Number of isolated finds recorded.
Type: For surveys, enter the applicable survey type such as Class I, Class II, Class III, or Paleontological. For non-surveys, enter the applicable report type such as Excavation, Monitoring, Historic Structure Assessment, Preservation Plan, etc.
Sites_IFs (Sites or Isolated Finds): Are there sites or isolated finds (IFs) found or recorded in the project area? Yes or No.
AGENCY_ (Lead Agency): Name of the Agency that the work was conducted for. Include a field office and/or agency project number if applicable (Ex: BLM – RGFO CR-RG-20613). If this is an addendum to a previous survey it should read "CR-RGFO CR-RG-20613 Addendum."
Rcrd_Date (Recording Date): Provide the date listed on the document.
SOURCE (Source): Enter the organization requesting the number. This can still be the lead agency (example: BLM – RGFO).
ZONE (Zone): Enter the Universal Transverse Mercator (UTM) Zone the project resides in (Zone 12 or 13).
X (X Easting Coordinate): Enter the X UTM center point coordinate of the survey. Latitude and Longitude coordinates will not be accepted. Leave blank if this is not a survey.
Y (Y Northing Coordinate): Enter the Y UTM center point coordinate of the survey. Latitude and Longitude coordinates will not be accepted. Leave blank if this is not a survey.
Acres: Enter the acreage of the surveyed area. Leave blank if this is not a survey.
