You are encouraged to report incidents even if you are not certain that they need to be reported.

The disclosure officer must report the following unless the officer is reasonably certain that a violation did not occur:

FTI incidents: Within 24 hours of discovery of an incident, report to the IRS Office of Safeguards at safeguardreports@irs.gov and Treasury Inspector General for Tax Administration (TIGTA) at the number found on their site:  https://www.treasury.gov/tigta/oi_office.shtml.

SSA incidents: Within 1 hour of discovery of an incident to the OIT CISO and the SSA Regional Office Contact at Heather.Hillmann@ssa.gov. If neither is available, the Disclosure Officer will contact: SSA’s National Network Service Center at (877) 697-4889 or the local SSA field office at (303) 844-7026.
